Patient Requirements
For a notarization to be valid, the patient (signer) must:
- Be conscious and alert — able to communicate with the notary
- Be mentally competent — understand what they are signing
- Sign voluntarily — not under duress or coercion
- Have valid ID — or credible witnesses if ID is unavailable
What to Have Ready
- The document(s) to be notarized (printed, not signed yet)
- Patient's valid photo ID (driver's license, passport, or state ID)
